How Clinical Pharmacist–Led Diabetes Care Changes Lives

Diabetes management isn’t just about numbers on a chart. It’s about understanding, support, and meeting patients where they are. At Baltimore Medical System, our Diabetes Management team plays a critical role in helping patients take control of their health through personalized education, medication management, and ongoing support. Led by Clinical Pharmacists working closely with providers, our approach blends evidence-based care with real-world problem solving—and the results speak for themselves.

What Our Diabetes Management Team Does

Our Clinical Pharmacists meet directly with patients to assess both medication knowledge and adherence, ensuring patients understand what they’re taking, why it matters, and how it fits into their daily lives. From there, we:

  • Educate patients on their medications and how they reduce the risk of diabetes-related complications

  • Promote adherence by helping patients create realistic plans, including setting phone alarms, simplifying regimens, and using pill organizers

  • Evaluate medication effectiveness and recommend adjustments to providers to help patients achieve their A1c goals

  • Coordinate with providers to ensure medications, supplies, and continuous glucose monitors (CGMs) are covered and accessible

  • Support broader health goals by reinforcing education around diet, exercise, and preventive care like immunizations

  • Follow patients over time to assess how medications and lifestyle changes are impacting their health

  • Empower patients with tools, knowledge, and confidence to actively manage their condition

This work is deeply collaborative—and deeply personal.

Real Patients. Real Progress.

One patient came to us with an initial A1c of 13.3%. Because of his family situation, his meals were largely limited to fast food. Rather than asking for the impossible, our team worked with him to make better choices within his reality—reviewing menu options and identifying healthier alternatives when dining out.

To support medication adherence, the Clinical Pharmacist helped him set alarms on his phone and created a clear, manageable medication schedule. Six months later, his A1c had dropped to 6.8%. At his visit, he shared how proud he felt of himself. His wife attended the appointment as well, and together they thanked our team for the guidance and encouragement that made the change possible.

Another patient arrived newly diagnosed with an A1c of 11.6%, overwhelmed and full of questions. Our team worked closely with his provider to ensure his medications and CGM were covered, removing financial and access barriers early on. The Clinical Pharmacist gave him her direct contact information, answering questions as they came up and providing reassurance along the way.

Within weeks, he felt confident managing his diabetes independently. His A1c dropped to 8.5% at three months, and today sits at 7.5%. When asked about his experience, he said:

“I don’t mind providing feedback if it will help you—because you helped me so much. I love working with you. You have been very helpful, and I wouldn’t have been able to do this without your help and support. I appreciate you.”

Other patients tell similar stories:

  • A patient who began care with an A1c of 10.8% saw it fall to 6.3% after medication adjustments to improve both effectiveness and side effects.

  • A patient who started at 13% in the spring reached 6.7% by December, maintaining adherence through counseling, medication optimization, diet, and exercise.

  • A patient with an A1c of 13.8% and multiple medication allergies required extensive review of past treatment history. With careful planning and documentation, our team helped him safely reach an A1c of 7.5% within months.
