Affordable healthcare for all

We will help you, even if you can’t pay

We make it easy and affordable to get the care you need. Our Health Benefits Advisors can tell you if you qualify for discounts. We can also help you apply for free or low-cost programs. If you are uninsured, we can help you apply for medical coverage. For more information on payment (English and/or Spanish), click here.

Sliding fee scale discounts

A “sliding fee scale” means that we can adjust the cost of your office visits and medications. Please click here to view our sliding fee application. The cost changes based on the size of your family and your household income.

Sliding fee is based on income and family size(e.g., yourself, spouse, domestic partner, children under 18; and parents, grandparents and adult children, if applicable). Here is our sliding fee schedule.

To apply, you must provide the following:

  • A valid ID for all family members applying for the discount

Examples include, but are not limited to:

  • Driver’s License
  • State ID card
  • Photo ID from Casa de Maryland
  • Official School Enrollment Letter
  • Consulate Cards

And one of the following:

  • Last month’s worth pay stubs
  • Award or benefit letter from the government
  • Letter from employer on company letterhead or employer statement stating wages
  • Two unemployment stubs

All forms of income includes: earnings, unemployment compensation, workers’ compensation, Social Security, Supplemental Security Income, public assistance (e.g. Temporary Cash Assistance), veterans’ payments, pension or retirement income, interest, dividends, rents, royalties, income from estates, trusts, educational assistance, alimony, child support, assistance from outside the household, employer statements and other miscellaneous sources. Noncash benefits (such as food stamps and housing subsidies) do not count.

If you do not have these items, call us for other options. You can call any of our centers at the phone numbers listed below.

How to get insurance

Are you insured? The best way to cut costs is to sign up for insurance. Are you uninsured? We can teach you about health insurance plans offered by the Affordable Care Act or Medicaid.

Our counselors can help you understand what is available and help you sign up for Medicaid. We will do this for anyone – not just our patients.
