CHILDHOOD AND FAMILY OBESITY
The social determinants of health (e.g. food deserts, toxic stress and minimal exercise at school and home) are conditions that create health disparities in our communities. A new pilot, BMS FIT will add obesity-related services and a preventive fitness program to BMS School-Based Health Centers in Baltimore City to reduce the risk of early onset chronic disease.
TELEHEALTH FOR SUBSTANCE-USE DISORDER AND MENTAL HEALTH
Physical space limitations in our School-Based Health Centers and limited access to behavioral health providers, restrict our ability to increase behavioral health services for Baltimore City students. The addition of Telehealth allows BMS to offer expanded behavioral health and obesity-related services for students in our school-based health centers.
CHRONIC DISEASE MANAGEMENT FOR DIABETES
Diabetes, an increasing source of mortality, is a significant concern for BMS with 14.3% of our adult patients diagnosed with the disease. A newly assembled BMS Chronic Disease Management Team has redesigned the delivery of chronic care management for diabetes. The team includes the patient, provider, clinical pharmacist and nurse. This model combines medication and lifestyle modifications promoting self-management with follow-up and testing closely monitored by the team.
