BMS Kicks off National Health Center Week

In recognition of National Health Center Week and Social Determinants Day, Baltimore Medical System would like to highlight our Director of Population Health and our Community Health Workers.

Agathe Hoffer-Schaefer joined BMS as the Director of Population Health in August of 2018. As the Director of Population Health, Agathe focuses on the improvement of health outcomes and quality and value of care for our patients. Agathe hails from Connecticut. She has dual BS degrees in Psychology and Forensic Science from Virginia Commonwealth University and received her MPH from The George Washington University. She believes everyone should have equitable access to healthcare, health opportunity, and optimum health attainment.

Sui Boriang is the Community Health Worker at our Highlandtown Health Center. She works closely with patients to address barriers improving their quality of life. Being a Community Health Worker is so much more than a job to Sui. It has given her an opportunity to meet many vulnerable patients and learn their stories. With the support of providers, other staff members and community stakeholders, Sui provides community resources, informal counseling, social support and advocacy to BMS patients.
